Re-Enrollment of Current Students – FACTS Family Portal Parent Web Instructions
Family Portal Link – In addition, families may go directly to factsmgt.com. From there, they can click on Family Login and then select FACTS Family Portal. The login screen showing the district code (CC-OH), username, and password fields will open as a pop-up window. If the family member or student has pop-up blocker enabled in their browser, this pop-up will not open, and they will need to adjust their browser settings to allow pop-ups for https://schoolsitefp.renweb.com/. If you are new, you will have to create an account.

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992.
